When foreigners report above cases in Japan, please inquire at the City Hall in advance. Procedures and necessary documents vary depending on the country.
The City Hall will guide the procedure based on the laws of each country after confirmation of the nationality and the details of the case. Regarding any reports and application to your native country, please inquire at the embassy or the consulate of your native country.

Reporting the Death
When to Report Death
You have to report death with the hospital death record (“Shibo Shindan-sho”) issued by the hospital within 7 days.
Things to Bring
Hospital Death Record, National Health Insurance Card (“Kokumin Kenko Hoken Sho”) if you have.

The “Certificate of Matters” of the notification form used for reporting to the home country,
etc. is issued only at the Citizen’s Division (Main Office) of the Kakegawa City Office. (350
yen per copy).
